After completing medical school, he began working as a neurosurgery resident at the famous Johns Hopkins Hospital in Baltimore in 1977. He demonstrated early on that he has good eye-hand coordination and is a skilled surgeon.
Because there were not enough neurosurgeons with experience in the field at the time, he was asked to join the Sir Charles Gairdner Hospital in Perth, Australia, in 1983. Carson accepted the opportunity despite his initial hesitation to travel so far from home.
In 1984, he returned to Johns Hopkins and was appointed head of pediatric neurosurgery at Johns Hopkins Hospital, as well as co-director of the Johns Hopkins Craniofacial Center, at the age of 33.
