Kieran began his swimming career in high school, where he was a member of his school team as well as the Ridgefield aquatic club, where he was named a NISCA All-American twice in a row. By 2017, he had joined the US National Junior squad and had begun competing in several global Junior events. In 2017, he won a silver medal in the 200m race at the World Junior Championships. By the end of his senior year, he was ranked eighth among NCAA recruits and had verbally committed to the University of Florida. He joined the University’s college team, the Gators. He began competing in SEC tournaments, and in 2018, he finished second in the 200m freestyle event.

In the same year, he finished eighth in the 200m freestyle and seventh in the 400m at the NCAA. He also ran in the 800m relay. During his sophomore year of college in the 2019-20 season, he became the fourth-fastest performer of all-time in the 200m freestyle and helped his team to the SEC 2019 championship. He also set an NCAA record in the 500m freestyle and finished third in the 100m in a career-best time. Kieran Smith also competed in the Phillips 66 national championships that year, finishing in second place. He was voted SEC Male Swimmer of the Year in 2020 after winning the 500m, 400m, and 800m freestyle events.

In 2021, he raced in the US Open Water Championships, finishing seventh, and in the TYR Pro Swim series, he was a champion and first in the 200m Freestyle.
